Document Request Form  

During the first semester of each year, the counseling office processes well over 1,400 college applications. To make the process go smoothly and to help us keep track of everything, this form must be filled out and submitted each time you need a transcript or other items submitted in support of your college applications.

During peak periods it may take up to two weeks after you submit your document request form before the materials are submitted. To reduce the possibility of confusion, please do not submit a document request form with a partially completed application.

Teacher Rec Questionnaire

If a teacher has agreed to write a letter of recommendation on your behalf you must provide background information to help make the teacher's job easier--and your recommendation better. Download this document, word-process it (please don't handwrite it), have it signed by your counselor, and nicely ask your teacher while also providing a copy.

UK Teacher Rec Questionnaire

If you will be applying to the UK, you must download and complete this form. UK references are submitted to UCAS and are written differently than US recommendations.

Applying to Summer Program

Please fill out this form completely each time you are requesting materials be sent in support of a summer school application. The Counseling Office uses this form to track which documents should be submitted with each application.

Applying to New High School

If you will be transferring from SAS and are applying to attend another high school, please fill out this form completely so that we can forward your transcript and other relevant documents to your new school. Official transcripts include a signature and seal and must be sent directly from SAS to the receiving school.

To provide the most helpful recommendation, your counselor needs as much information as possible. Unlike the teacher recommendation background form which is a downloadable Word document, the counselor background is found on Family Connectiom. It is not available until after July 1 between the junior and senior year since it asks questions about how you spent your summer. Seniors should login to Family Connection and complete the "my counselor rec" document online. The more information available about you, the easier it is for your counselor to include anecdotes to support your application.

Certification of Finances (for visas)

If you plan to study in the US, you will not be granted a visa unless you can prove you have sufficient financial resources to pay for your college, living expenses, and a return trip to your home country. You can use this form and attach a document from your bank. 

High School Profile

Each time a transcript is sent to a college or high school a "school profile" is included. The profile is updated annually.

SAS College Guide

In December, each junior is given a printed copy of our annually updated College Guide. You can also download a copy from this website by clicking the link above.
